The individual hired for this position will provide brief, virtual, follow-up interventions following inpatient hospitalization, and facilitate patient engagement in outpatient care. They will also play a key role in the quality improvement initiative, and will collaborate with patients, their families, members of the multidisciplinary BHUC/Access team, and Inpatient Social Workers.
