deterioration, sentinel events, and/or poor outcomes. The care manager works with the ambulatory social worker and community health care worker as an integral part of the complex care management team to ensure that that care for the most vulnerable patients is coordinated and managed. These patients must receive optimal care including acute illness management, chronic disease management, and preventive care across multiple health settings and multiple physicians/providers.
- Graduate of an NLN-accredited School of Nursing, BSN preferred
- Licensure: MA. RN Licensure required
- Certifications: Current AHA (American Heart Association) or ARC (American Red Cross) BLS Provider Certification required
- Work Experience: Minimum of 5 yearsof broad based clinical experience
- Demonstrated competency in managing a caseload of ethnically diverse patients across the continuum of care and working effectively with
- patients with substance abuse and behavioral health conditions required.
- Previous Care Management experience strongly preferred
- Previous experience working with safety net population preferred
- Previous experience working across a variety of settings in the care continuum (including ambulatory and community health nursing) preferred
