Cambridge Health Alliance (CHA), a well-respected, nationally recognized and award-winning public healthcare system is seeking a full time Clinical Specialist II, Nurse Practitioner, to join our CHA PACE Program. CHA PACE is a Program of All Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE’s high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services. 

We are seeking a dedicated and experienced Psychiatric Nurse Practitioner (PMHNP) to join our highly functioning interdisciplinary team. In this role, you will provide comprehensive psychiatric assessments, diagnoses, and treatment plans for elderly patients, focusing on their mental health needs in participants’ home, assisted living and nursing home settings. Responsibilities:

  • 36 Hours/week, M-F, MNA Cambridge Union (32/40)
  • Conduct psychiatric evaluations and assessments for frail older adults who meet nursing home level of care in community settings (homes, assisted living, day center, nursing homes).
  • Develop and implement individualized treatment plans.
  • Monitor and manage medication regimens.
  • Closely collaborate with interdisciplinary teams, including primary care providers, social workers, and caregivers.
  • Provide education and support to patients and their families regarding mental health issues and treatment options.
  • Maintain accurate and timely documentation of patient care.
  • Participate in case reviews and quality improvement initiatives.
  • Maintain current awareness of new concepts, methods, techniques, regulations, etc., related to the Nurse Practitioner role. 
  • Communicate effectively and work as part of a team. Displays ability to get along with co-workers, other departments and the public. 
  • Perform other related or similar duties as directed or as responsibilities necessitate.
  • Maintains current Basic Life Support “C” Certification. Certification in Psychiatric Mental Health Nursing by the American Nursing Association.

Qualifications:

  • Must be a graduate of an accredited Nurse practitioner program.
  • Master’s degree in Nursing with a specialization in Psychiatric Mental Health or equivalent experience.
  • Current PMHNP certification and licensure in Massachusetts.
  • MA. RN Licensure required
  • Current Massachusetts licensure is required authorizing practice in an expanded role as well as current DEA number and Mass DPH registration to prescribe controlled substances.
  • Current Massachusetts licensure authorizing practice in an expanded role as a Psychiatric Nurse Clinical Specialist and authorization including prescriptive authority.
  • Enthusiasm for doing home visits with reliable transportation.
  • Minimum of 2 years of experience in geriatric mental health preferred.
  • Strong understanding of mental health issues affecting the older adult population.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
