Department Description:
CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.
Job Summary:
In collaboration with the PACE Interdisciplinary Team (IDT), the Caseworker works within the PACE model of care to actively frame interventions for program participants' comprehensive care plans. They develop, coordinate, and implement discharge plans consistent with each participant’s individual goals and IDT recommendations, as well as document participant progress in the medical record. In addition, the Caseworker acts as a participant navigator for new enrollees and their families. By establishing empathetic, trusting relationships with participants, they act as a resource to the MSW staff in navigating and providing support for participants' concrete service needs.
Qualifications:
- BS in related health field or relevant experience
- Experience working with relevant community groups preferred; demonstrated experience working in multicultural and multidisciplinary settings required
- 1 year experience working with the geriatric population
- At least 2 years experience with Care Coordination, Discharge Planning, Utilization Review, or similar
- Bicultural and/or bilingual preferred in the specific cultural/language group of the population/community served
- Reliable transportation for home visits
- Flexibility, initiative, creativity, and excellent organizational skills
